The Northern Buckeye Health Plan (NBHP) is the Northwest Division of Optimal Health Initiatives (OHI). OHI was founded in 2007 and is currently one of the largest consortia in the state, maintaining a collective total of 44,000 participants. NBHP alone is composed of 29 participating members for the purpose of purchasing health, dental, and life insurance products at a reduced cost. NBHP partners with several venders in an effort to control both claims and administrative costs.

Employees must work at least 25 hours per week in a nine month per year assignment to be eligible for insurance benefits. You must inform the Business Office of any qualifying event that changes your insurance status within 30 days.

As a member you can log into your Allied account and print additional copies of your ID card. If you would like additional plastic cards please contact the Business Office and we can request those.

NBHP partners with Express Scripts for both retail and mail order prescription programs. To locate and Express Scripts pharmacy in your area, contact them at: (866) 275.0044 or visit them online at www.express-scripts.com
Requesting Additional ID Cards: As a member you can log in to your Express Scripts account and print additional copies of the id card. You can also contact customer service by calling (866-275-0044) to have additional cards sent via mail. Customer Service will most likely refer you back to your account for the cards to be printed.

NBHP partners with HealthSpan, LLC to provide Care Management programs and services. While providing services you have already had available such as maternity management, individual case management and utilization management (pre-certification), HealthSpan will also be able to provide disease management and personal health management services.

Requesting Additional ID Cards: As a member you will need to sign on to your Delta Dental account and print additional id cards. They do not send out additional cards once the originals are received.

Requesting ID Cards: No cards will be mailed out. As a member you can log into VSP, create an account, and print off a vision insurance card.

Sick/Personal/Vacation Days

Sick Days
Each regular full-time employee shall be granted sick leave in accordance with Ohio law and Board policy. Employees who render regular, part-time services shall be entitled to sick leave in proportion to the time actually worked. For those eligible, sick days may be taken in one-quarter (1/4) day increments.

Personal Days
The Board shall provide for an employee's absence for personal necessity. Up to three full days of unrestricted personal leave with pay may be used, if approved by the Superintendent or designee, each contract year by eligible employees. Personal leave is not cumulative. Personal leave days must be used in full or half day increments.

Vacation Days
Each regular classified employee serving 11 or more months per year shall be entitled to an annual vacation, with pay, based on length of service for the Northwest Ohio ESC. The Board reserves the right to specify the conditions under which vacation time may be taken. Vacation days may be taken in one-quarter (1/4) day increments.

Tuition Reimbursement

The Board, in an effort to encourage certificated staff to continue their education and enhance expertise, will provide partial reimbursement for course work taken while an employee of the Board. Please see your employee handbook for more information.

Make sure to turn in the Tuition Application form BEFORE starting your course, and turn in the Tuition Reimbursement Request form AFTER you have completed your course. The Business Office requires originals of the application and reimbursement request forms.
