Draft Data Collection Tool for Students with Disabilities (ODE Guidance for 2012/13)

Recent changes include:

  • Reporting of amendment dates when those amendments change Least Restrictive Environment (LRE), required test types or Ohio Graduation Test (OGT) consequences;
  • Reporting of an LRE setting of “correctional facility” when a student is receiving services in DYS, a detention center or a community-based facility; and
  • Reporting child-find activities and the creation of Individual Service Plans (ISP’s for students receiving services in non-public schools)

Senate Bill (S.B.) 316 continues the requirement for students with disabilities to undergo a comprehensive eye examination within three months of receiving special education services for the first time. A new reporting requirement was identified in S.B. 316 which requires ODE to collect and report data about the implementation of this law. Although the data collection mechanism to fulfill the reporting requirement has not yet been identified, OEC believes that data will be more accurate if districts maintain a record of eye exams throughout the school year. Below is a tool that will assist in that effort. There is no requirement to use this particular tool, but the information on it will be collected at the end of the 2013 school year and subsequent school years.
